*Our form allows for multiple bookings to meet ADA compliance laws, because we understand that some people are unable to reserve a vaccine for themselves.
With that said, we will be directly contacting anyone who reserves more than one appointment slot and confirming that the second (or third) reservation is for someone who is disabled or otherwise unable to use the appointment booking form due to disability.
After you sign up for COVID-19 vaccine appointment, please download and complete the Vaccine Consent Form, print and sign it, then bring it with you to your appointment.
Also, if you have a Red, White & Blue Medicare A/B card, please bring a copy of that card with you.
