In order to minimize confusion with our upcoming COVID-19 vaccinations, we have suspended the online bookings of our usual vaccine programs.
If you need a typical flu vaccine, you can still call our locations to schedule an appointment.
This is only temporary, and we will bring back online bookings soon.

After you schedule your appointment, please download and complete the Vaccine Consent Form, print and sign it, then bring it with you to your appointment.
Also, if you have a Red, White & Blue Medicare A/B card, please bring a copy of that card with you.
